I am always interested in this topic. I wish there was a tool to use at the high school level which could accurately measure the topics of strength and the learning gaps for individual learners. It would be so helpful to have a map of previous learning so that the gaps could be targeted and supported. Too often I find that students who are not ready or able to go on in math, are carted along because this type of individualized intervention is not available.

I'm also very interested in all the fascinating work that's being done in the areas of conceptual instruction for number sence. I was speaking with a collegue yesterday and inspired by her knowledge of how to use manipulatives and 1cm graph paper to illustrate addition, subtraction, multiplication and division in a way that more students would be able to connect to. I'd love it if teachers could link to some awesome resources here on conceptual instruction, or upload good 'math minutes' or other reinforcers that could be used at the beginning of a class.
